In order to facilitate the uniform reporting of critical incidents in all medical fields, the perioperative patient safety group of the UHBS, Departement of Anaesthesia in collaboration with the Swiss Medical Association (FMH) and the Swiss Nurse Association (SBK) have created the CIRSmedical^© system. It is based on the experiences with the above Anesthesia oriented CIRS system and has brought Critical Incident Reporting to all specialities:
